This candidate is a team spirited individual who takes initiative and enjoys providing evidence based medical nutrition therapy to individuals and groups in both inpatient and outpatient settings. The Clinical Dietitian working in the inpatient setting identifies patients at nutritional risk, performs nutrition assessments in accordance with facility standards, provides timely patient follow-up and monitoring of nutrition care plans, communicates pertinent information to nutrition care representatives to ensure meal service needs are met, educates patients / significant others, and participates in multidisciplinary patient care rounds. The Clinical Dietitian working in the outpatient setting assesses patient education needs, selects education materials based on individual needs, demonstrates effective counseling skills, evaluates effectiveness of interventions, revises interventions as needed, and provides reinforcement via follow-up visits. The Clinical Dietitian documents outpatient nutrition visits in accordance with facility standards, and maximizes productivity when scheduling follow-up visits.

We are currently looking for an Outpatient Clinical Dietitian in the Diabetes Center. Under minimal supervision, the Registered Dietitian is responsible for educating patients and their families on diabetes self-management skills and medical nutrition therapy. The Registered Dietitian works collaboratively with the Outpatient Diabetes Staff to ensure that all patients with diabetes that are referred to the Outpatient Program receive specialized training in diabetes management. The Registered Dietitian works collaboratively with all members of the health care delivery team to promote quality patient care. The Registered Dietitian must be able to communicate effectively with patients, families/significant others and members of the health care delivery team. The Registered Dietitian actively participates on the local and system wide Diabetes Committees and other hospital committees as requested.
Qualifications: Certification/License Requirements: Registered by the American Dietetic Association Licensed by the Commonwealth of Massachusetts Certified Diabetes Educator recommended (CDE)
Education/Experience Minimums: Education: Bachelor Degree in Science required Registered Dietitian Current RD licensure in MA

Full Time Diabetes Education Nurse - Exciting Opportunity for Innovation and Collaboration
The Diabetes Education Nurse will work in collaboration with health care and human service professionals to provide program development and patient focused care. The person who holds this position functions as an innovator and change agent in pursuit of improved nursing practice and improved patient outcomes. The Diabetes Education Nurse works collaboratively with VNA staff and other JF&CS departments on the planning, implementation and evaluation of services that will not only be implemented by JF&CS Staff, but presented to a wide audience, including individuals with diabetes, primary care providers, community partners, and/or payers. The scope of practice encompasses teaching and evaluation of patients admitted to the VNA as well as supporting diabetes education in the community.
Program Development: ·Develop structural foundation for chronic care management on which diabetes education will be built ·Program development in collaboration with other JF&CS departments, e.g. Nutrition and Social Services
Community Partnerships · Develop relationships within the community for diabetes self-management support; assess target populations and identify community partners/sites ·Create partnerships with physicians, insurers, other stakeholders in chronic disease management
Client-Specific DSME/T ·Engage individuals in active self-management; implement strategies to support behavior changes ·Collaboratively develops educational goals, learning objectives, and teaching methods with the primary care team; integrates DSME/T plan into the overall plan of care
EDUCATION AND QUALIFICATIONS ·The applicant must have a current Massachusetts’s RN license and be certified as a Diabetes Educator (American Association of Diabetes Educators, National Certification Board for Diabetes Educators) or meet the eligibility requirements for initial certification. ·Data analysis skills and program development experience are required; ·Computers skills consistent with ability use Point of Care technology for Electronic Medical Record and data analysis are required as well as competency in MS Office Suite applications.

Exciting Opportunity for an experienced Diabetes Nurse Educator!!
Manager, Clinical Education Programs
As a member of an interdisciplinary team in the Healthcare Services department of Joslin Diabetes Center, the Nurse in this position will be involved in the development and oversight of new products and services that extend the Joslin mission to a wide variety of audiences including primary care providers and individuals with diabetes. The individual will collaborate closely with both the various client groups (which may include pharma, payors, and industry) as well as with the clinicians and program staff of Joslin Diabetes Center. The position requires excellence in the following skills: · Managing client relations · writing for professional and patient audiences · awareness of current diabetes trends and research, particularly in education · Public speaking.
Some travel is required.
Patient care in Joslin Clinic will also be an element of the job, with the individual seeing patients, teaching classes and/or handling other responsibilities for up to 20% of the time.
MINIMUM REQUIREMENTS:
EDUCATION: Diabetes Educator (CDE preferred), Baccalaureate degree in nursing.
EXPERIENCE: Three years experience in direct diabetes care with emphasis on patient education. Skills in project management, client relations and leadership are desired. Excellent organizational, problem-solving, presentation and writing skills, with ability to work both independently and collaboratively on multiple tasks.
